Bariatric Surgery Patient Information
Dr. Van L. Wagner and our bariatric surgery team here in St. Louis have created a step-by-step process for our prospective patients to make your experience with us as easy and efficient as possible. If you are considering a weight loss surgery procedure such as LAP-BAND® Adjustable Gastric Banding, REALIZE® Band, Roux-en-Y gastric bypass, or gastric sleeve surgery to combat morbid obesity and start living a healthier, and slimmer, lifestyle, then we encourage you to review this helpful guide to getting started. Step One – Free Educational Seminar
Usually, the first step is to attend one of our free educational seminars. These informational sessions will help you understand the bariatric surgery process and which procedures may be right for you. They will also help you in understand the dietary and lifestyle changes that will be necessary following surgery.

Step Two – Medical History Form
If you’re ready to get started now, please completely fill out our 3-page Medical History for Bariatric Surgery form, and fax or mail it to the address listed on our Contact Us page. Once we have received your completed paperwork, you will be assigned a patient advocate who will contact your insurance, verify your benefits, ensure that you meet the criteria, and are a candidate for bariatric surgery.
Step Three – Schedule a Private Consultation with Van L. Wagner, MD or Kumaran Chinnappan, MD, FACS
After we have received your Medical History Form, you will be contacted by your patient advocate to schedule a private surgical consultation with one of our Board Certified Weight Loss Surgeons. The initial consult includes a complete history and physical examination; discussion of your weight loss surgery options and all aspects of the procedure you are considering; and overview of your specific insurance requirements and payment options. The consultation typically lasts from 30 to 60 minutes.
Step Four – Schedule Surgery
With Insurance | After all of your insurance requirements have been received by our office, we will submit the surgery pre-determination letter to your insurance company on your behalf. Once insurance approval is obtained, surgery is typically scheduled two-to-four weeks later. There is no wait-list for surgery – we will conduct pre-operative testing during this two-to-four week period.
Self-Payment | If you’ve elected to self-pay for your procedure, we will work with you to schedule surgery according to your desired timeline.
